5. A statement that the information in your notice is accurate and, under penalty of perjury, that you are the copyright owner or authorized to act on behalf of the owner.

Once we receive a complete and valid DMCA notice, we will review the request and take appropriate action, which may include removing or disabling access to the content in question.
